How long has the agency been in operation?

Patient Care has been providing excellence in home health care since 1975. Over the years, the company has expanded into serveral states through both new office openings and acquistions.

What services are available?
At Patient Care, our goal is to help you or your loved one remain at home, living as independently as possible. Whether you need assistance for just an hour or two weekly, or on a daily or live-in basis, Patient Care can respond immediately, providing qualified, dependable staff. In each state, licensure and/or certification dictate the types of services that an agency can provide. How does the agency determine which services are needed?
During the initial point of contact with Patient Care, whether by phone or home visit, a Patient Care nurse will work with you to identify your individual needs. Based on a thorough assessment, the nurse will determine/recommend the most appropriate level of care to allow you to remain safely in your home while meeting your daily needs. When appropriate, the Patient Care nurse may also recommend the appropriate length of service.


Is the agency licensed and accredited? Explain.
Patient Care is a licensed, accredited and, in most of our locations, Medicare certified home care company.

In most states there is some form of licensure for the provision of home care services. Licensure will dictate the type of services an agency can provide and will often require that an agency establish policies and procedures associated with the provision of those services. The state will conduct announced and unannounced survey visits to determine if state regulations are being met.

Accreditation is obtained on a voluntary basis. Similar to a state survey, accrediting bodies will make both announced and unannounced site visits to determine if the agency is meeting the standards and guidelines set forth by the accrediting organization. Home care agencies are typically accredited by:

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO)
and/ or Community Health Accreditation Program (CHAP).


What geographic areas does the agency serve?

Our staff is available twenty-four hours a day, seven days a week.


Will my insurance plan, Medicare or Medicaid cover the costs for care?
Services may be covered by Medicare, Medicaid, Private Insurance, HMOs and Worker’s Compensation.


How are employees screened prior to employment?
At Patient Care, we take pride in our tradition of excellence. Patient Care staff, as a condition of their employment, must adhere to formal guidelines for conduct, established by our Quality Management Department.

We are highly selective in the candidates we accept into our training program for Home Health Aide certification. Candidates must demonstrate sincerity, maturity, a professional demeanor and a well-groomed appearance. Prospective employees undergo a rigorous screening process, a personal interview, verification of all employment history and references, and a physical exam. Routine tuberculosis screens are held for all staff.

What kind of training do employees receive?
All of our nurses receive extensive home health care orientation and in-service training.

Our Home Health Aide training program, led by a Nurse Educator, consists of a wide range of subjects from emergency care and procedures to anatomy and physiology.

In order to graduate from the program and gain certification, students must pass an intensive written test and competency exam. In addition to successful completion of this comprehensive curriculum, home health aides are required to attend a specified number of inservice programs each year, on a variety of topics relating to the care of patients. Home Health Aides must also be in compliance with Patient Care’s established rules of conduct.


Who supervises the employees? How frequently?
All staff are under the regular supervision of a Nurse, who assesses the patient, monitors care and confirms patient satisfaction.
