Geriatric Case Management
Geriatric Case Management is conducted by our trained
professionals who specialize in working with older adults. Their understanding
of the aging process is invaluable in helping older adults and their families
make informed decisions to help the elderly maintain a satisfying quality
of life.
Geriatric case managers may be involved in these activities:
- evaluate the needs of the elderly in the home
- identify community resources including recreation programs, in-home support, nursing homes and home healthcare
- find alternative living arrangements
- arrange, coordinate and monitor services and care needs<
- provide education and counseling .
